Mr Clark
Mr Clark had been living at home and cared for by his family. He was admitted to the hospital with a severe infection and in what was described as an “extremely neglected state”. After several weeks in hospital, his condition had improved enough for him to be discharged, but he was assessed as needing 24-hour care and lacking the capacity to make decisions relevant to his future welfare. Family members wanted to take Mr Clark home, offering to manage all his care.
A safeguarding inquiry was instigated as there were unfortunately concerns that the family would be unable to provide sufficient care for his needs. The relationship between family members and health professionals became extremely strained.
The Family Group Conference meeting provided an opportunity for the family to fully understand the extent of care that Mr Clark required and enabled them to explore their options together as a family. They were able to conclude that nursing home care was in Mr Clark’s best interests and find ways to still provide support themselves in this setting. Whilst Mr Clark was not able to be present, he was represented by an independent advocate – a key consideration when someone isn’t able to be present to ensure that their voice and interests remain heard.
