Helping a young adult reduce readmissions

Upon referral, we worked with a young adult and their family following repeated admissions to hospital due to self-harm and challenging behaviours.

Working together with family, and professional services, the Family Group Conference (FGC) enabled a plan to be put into place that improved relationships and addressed behaviours.

A Review showed positive family relationships being made and reduced admissions.
